Can an Ultrasound Be Normal When You Have Endometriosis?

A normal ultrasound does not answer every pelvic-pain question

Ultrasound can identify ovarian endometriomas and some forms of deep endometriosis, especially when performed and interpreted by experienced clinicians. Superficial disease may not be visible. Therefore, a normal scan does not by itself prove that endometriosis is absent.

It also does not prove that endometriosis is present. Symptoms can arise from several gynecological and non-gynecological conditions.

What imaging is useful for

Imaging can assess the uterus, ovaries, fibroids, adenomyosis, cysts and selected signs of deep disease. MRI may be helpful in particular complex situations, but it is not a universal next step for every patient with pain.

Diagnosis is a clinical process

The pattern, severity and timing of symptoms; examination; response to previous treatment; fertility goals; and imaging all contribute to the plan. Laparoscopy can confirm endometriosis and may allow treatment, but surgery is not automatically required merely because symptoms exist.

Ask what conditions remain possible, whether the scan was designed to assess endometriosis, whether initial medical treatment is reasonable, what would justify further imaging or laparoscopy, and which warning signs need faster review.
